Intermediate Care Nurse (IMC RN)

• Assessment & Intervention: Conduct thorough assessments, recognize changes in patient conditions, and implement appropriate nursing interventions to stabilize and improve patient outcomes.
• Care Coordination: Collaborate with a multidisciplinary team, including physicians, respiratory therapists, and social workers, to develop and implement individualized care plans for patients.
• Patient & Family Education: Educate patients and their families about the care process, treatment plans, and recovery expectations to ensure they are well-informed and prepared for discharge.
• Accurate Documentation: Maintain detailed and accurate documentation of patient assessments, treatments, and responses in the electronic health record (EHR) system to ensure high-quality and consistent care.
• Continuous Learning: Stay updated with the latest evidence-based practices and advancements in intermediate care nursing through ongoing education and professional development.
• Mentorship & Leadership: Support and mentor new nurses and nursing students, fostering a positive and collaborative learning environment.
